APPLICATION PROCESS
  2010-2011

Enrollment begins February 15, 2010 for FBA Families and
All K4 Applications currently being accepted

Enrollment opens to the public on March 1, 2010

The following "application process" is for first time enrollment and must be fully completed before a student is enrolled.   Information and forms for re-enrollment are made available for families in the spring each year.

• Criteria for Admission:  Admission criteria must be read and signed by parent or guardian prior to assessment.

• Age Criteria:  Students in the Three Year Old program must be three by September 1 and must be potty trained. K4 students must be four years of age by September 1st.  K5 students must be five years of age by September 1st.
  
  
• Assessment:  1) Appointment for evaluation testing (grades K5 thru 9th) must be made and $25 fee paid. 2) Date of birth must be verified with a certified birth certificate.  3) Copy of student's permanent records may be submitted at this time or requested by the registrar from the last school attended.
  
  
• Application Forms:  ALL application forms must be completed and returned to the office.
  
  
• Review of Records:  1) Final assessment is made by the Administrator.  2) 6th, 7th, 8th, 9th, 10th, 11th 12th grade students and their parents must meet with the administrator.  3) The registrar will notify parents of assessment results.
  
  

• Enrollment Fee:  The enrollment fee is due at time of registration. This is a non-refundable fee, which secures a student's place in the Academy.
